一位初次診斷直腸癌病患手術後的護理經驗

Nursing Experience of a Patient with First Diagnosed Colon Cancer During Post-operative

本文是探討一位直腸癌的病患，初次被診斷直腸癌手術後的護理經驗，筆者在2006年3月15日至3月30日護理期間藉由直接照護、觀察、治療性會談、身體評估等方式進行資料收集，運用Gordon 十一項功能型態做為評估指引，發現個案有疼痛、預期性哀傷、營養狀況少於身體所需、及身體心像紊亂等問題，護理過程提供疼痛處理、瞭解疾病造成的感受、提供飲食指導、及教導參與腸造口的照顧，使得個案能接受自己身體外觀的改變，並增進個案對疾病的認知及自我的照顧調適能力。期望藉此護理經驗之分享，能提供護理人員臨床照護之參考。

This case study reported the nursing experience for a patient with the first diagnosed colon cancer during post operative period. The duration of nursing care by author was between March 15 and March 30, 2006. The Gordon functional health assessment guide was used to assess nursing problem through direct care, observation, and systematic interview, and physical assessment. Nursing problems included pain, anticipatory grieving, nutrition altered: less than body requirements, and disturbance body image. Nursing intervention programs included pain management, understanding of illness perception, dietary direction, and education of colostomy care. The patient finally accepted his body appearance and effectively adapted for illness cognition and performed self-care. This experience provided nursing staff with a reference for caring similar patients in the future.
