提升外科加護病房留置口內氣管內管病人口腔黏膜完整性之改善專案

Promotion of Status of the Oral Mucosa in Surgical Intensive Care Unit Patients with Oral Endotracheal Tube

口腔衛生與生活品質有高度的相關，提供住院病人口腔護理是最基本的護理活動之一。本專案旨在提升外科加護病房留置口內氣管內管病人口腔黏膜完整性。現況問題為護理人員未執行口腔黏膜完整性評估及執行留置口內氣管內管病人口腔護理一致性低，導致病人口腔黏膜完整性偏低。經由建立「留置口內氣管內管病人口腔黏膜完整性評估表」作為評估工具、擬訂護理人員執行「留置口內氣管內管病人口腔護理作業標準」及舉辦「留置口內氣管內管病人口腔護理教育訓練課程」之在職教育，執行成果為病人口腔黏膜完整性由39.83％提升至61.46％。期藉由本專案推動之經驗，增進臨床護理人員對留置口內氣管內管病人口腔黏膜完整性之瞭解並提供口腔護理作業標準作為執行之參考，以提升加護護理專業價值。

Oral care for patients with oral endotracheal tube is a common and necessary nursing activity in the intensive care unit (ICU). It decreases ventilator associated pneumonia and decreases the complications of the oral mucosa. The aim of this project is to improve the completeness of the oral mucosa in surgical ICU patients with oral endotracheal tube. By using “Evaluation form for the oral mucosa in patients with endotracheal tube”, we established the standard procedure of oral care for patients with oral endotracheal tube. Educational programs and individual education system of “Nursing Care of the Oral Mucosa for Patients with Oral Endotracheal Tube” improved the status of surgical ICU patients with oral endotracheal tube from 39.83% to 61.46%. This article provided a standard procedure of oral care for patients with oral endotracheal tube and emphasized the importance of such care.
