膽石性腸阻塞

Gallstone Ileus

膽石性腸阻塞，由膽石阻塞腸道而產生的機械性腸阻塞，是非常罕見的疾病，但在文獻有詳細的記載為老年人腸阻塞的一種原因。雖現在科技在診斷學上不斷進步，此病常被誤診，以致常常延誤治療，因此在術前和術後的照料上有非常高的發病率和死亡率。這是一個膽石造成的空腸阻塞個案，病人於住院後第二天經由小腸攝影診斷出來。病人並且接受切腸手術和膽道廔管縫合手術。

Gallstone ileus is a mechanical intestinal obstruction caused by impaction of one or more gallstones in the lumen of bowel. This condition is unusual but a well-documented cause of intestinal obstruction in the elderly. Despite advances in diagnostic modality and improved imaging study, the diagnosis is frequently missed or unduly delayed. Preoperative and postoperative management of gall stone lieus is associated with high morbidity and mortality rate. This is a presentation of gallstone obstruction at level of distal jejunum that was recently treated in our institution. Thu gall stone impaction was diagnosed by upper G-I series with meglumine diatrizoate (Gastrografin) swallow. He received operations of segmental jejunum resection and cholecystoduedenal fistula closure sucuerrfully.
