社區健康促進計畫之評價

Evaluation of a Community Health Promotion Program

長久以來衛生局所及醫療院所推動社區健康營造，常以宣導、講座及辦活動之方式，進行健康知識的傳播。缺乏長期追蹤及實證資料來證實計畫的有效性。有鑑於此，筆者自2011年開始協助某醫學中心推動轄區內社區健康促進評價計畫。以社區內代謝症候群高危險群個案為對象，和閒世界衛生組織的健康促進策略、台灣社區認證標準及Donabedian的評價理論，執行社區健康促進評價計畫，期望能提供未來醫院或社區在推動社區健康促進計畫評價之參考。

Health bureaus and health care facilities often promote community health through community health education outreach using advertisement, seminars, and events. However, demonstrating the effectiveness of these promotion plans requires long term follow-up and evidenced data. Therefore, the author initiated an evaluation plan for the community health promotion of a medical center within its district in 2011. In this study, the subjects were the population at high risk for metabolic syndrome. The author applied the health promotion strategies of the World Health Organization (WHO), the identification standard of that community in Taiwan, and Donabedian's evaluation theory. The author expects that this study can be used as a reference for other hospitals and communities that plan to promote community health in the future.
