This paper shares a nursing experience of applying a transtheoretical model to change a patient's knowledge on diabetics diet. During the nursing care period from February 3 to May 3, 2012, the author collected data through a comprehensive nursing assessment, observation, interview, and medical chart review. The patient's major health problems were a lack of knowledge on diabetic diet and poor self-monitoring of blood sugar levels, which led to the patient's poor control of blood sugar. Upon applying a transtheoretical model for behavior change, the case entered the preparatory stage of behavioral change. Following each distinct stage, the author offered relevant knowledge and strategies for behavioral change. Through experience sharing with a diabetic support group, a diabetic video presentation, the use of a diabetic diet table, and set contract on behavioral change with the patient, the patient successfully modified diet habits and cooking skills to fit a diabetic dietultimately achieving the goal of blood sugar level control and maintenance. |
