Medication errors may cause patients severe harm or even death. Health care providers and professionals should take precautions with patient medication safety to promote the effectiveness of medication in illness recovery. This project was designed to promote instruction rates of medication for patients in the surgical ward. The major factors responsible for poor medication instruction rates were the nurses' poor knowledge about medications, a lack of in-service on medication, a lack of patient instruction sheets, a lack of a standardized procedure for inpatient education, and an absence of a monitoring mechanism. The project included educational reinforcement for nurses, development of medication instruction sheets by the pharmacy and the informational technology department, and the establishment of a standardized operational procedure and monitoring mechanism for medication education for surgical inpatients. After implementation of the project, the patient medication instruction rate significantly increased from 72.6% to 94.0%. The score on nurses' awareness of medication improved from 73.3 to 96.7. Patient satisfaction scores on medication instructions, including explanation of medication reactions, adverse effects, and warnings, increased from 84.4% to 91.4%. The success of the project indicates that effectiveness in patient care can be improved through multidisciplinary collaboration. |
