照顧一位首次接受血液透析患者之護理經驗

A Nurseing Care Experience for First Time Hemodialysis Patient

本個案報告是在描述照顧一位首次接受血液透析治療之護理經驗。護理期間101年8月18日至24日，筆者運用Gordon十一項功能性健康型態進行護理評估，並藉由身體評估、觀察、會談及病歷查閱方式收集資料，確立個案主要健康問題為體液容積過量、營養知識缺失、心理調適障礙。在照護過程中，依個案之個別性提供血液透析相關知識、腎友的經驗分享、飲食指導，也助其維持正常透析生活，提升自我照顧能力，教導個案及家屬將腎病飲食及注意事項融入日常生活中，協助個案以正面的態度來面對後續治療，讓個案能成功適應長期血液透析治療。

This paper describes a nursing experience of caring for a patient who received the first hemodialysis. The nursing period was from August 18 to 24, 2012. The author applied Gordon's 11 Functional Health Pattern Assessment Tool for patient assessment. Data were collected through physical examination, observation, interview, and medical record review. The identified major health problems were fluid volume overload, knowledge deficiency, and maladaptation. The author provided the patient with educational material about hemodialysis and a renal diet. An experienced dialysis patient was arranged to meet with the patient to share experience and provide support. The author assisted the patient in maintaining a regular lifestyle with dialysis through self-care skill enhancement, such as a therapeutic diet and discussing things the patient ought to know about daily living. The patient was encouraged to embrace a positive attitude toward lifelong dialysis with the expectation of a successful adaptation to lifelong dialysis treatment.
