提昇門診說明大腸癌篩檢轉介單之正確率

Project to Improve the Correct Rate of Explanation of Colon Cancer Referral Sheets in Outpatient Clinic

為配合衛生福利部國民健康署大腸癌篩檢補助計畫，本院推動大腸癌篩檢，以達早期發現早期治療目的。2011年3月7日至11日發現護理人員在門診大腸癌篩檢轉介單之發放率51.4％，3月14日至25日間，經專案小組實地查看，發現門診大腸癌篩檢轉介單說明正確率僅30.2％。經由現況分析，得知正確率低之因素為「缺乏篩檢轉介單作業指引」、「無相關在職教育訓練」、「無查核制度」，以及「教學內容不一致」等四項。藉由制定門診大腸癌篩檢轉介指引單、規劃教育訓練、制定內容範本，以及建立稽核制度等措施後，本院門診大腸癌篩檢轉介單發放率提昇至100％，且該轉介單說明正確率提昇至91.9％；顯示本專案有效提昇門診發放大腸癌篩檢轉介單，頗值得相關單位參考與推廣。

With the Bureau of Health Promotion grant program of Colon Cancer Screening, the purpose of this screening is to achieve early detection and early treatment of colon cancer. According to statistics in 2011, the distribution rate of colon cancer referral sheets was 51.4% and the correct rate of explanation of colon cancer referral sheets was only 30.2%. The related factors of low correct rate included (1) the lack of guidelines of colon cancer referral, (2) no job training program, (3) no evaluation protocol, and (4) inconsistence of teaching contents. By conducting this promotion project, we developed the guidelines of distributing colon cancer referral sheets, on-job training program and teaching content as well as evaluation system. After completing this project, the rate of distributing colon cancer referral sheets was increased to 100%. The correct rate of explaining referral sheets also was raised to 91.9%. These results showed that the project effectively improve the referral process of colon cancer screening. Information of this project may serve as future reference for healthcare providers and nursing professionals in the improvement of colon cancer screening.
