以矯正植體扶正舌側傾倒的下顎第二大臼齒－－病例報告

Correction of a Lingual Tipped Mandibular Second Molar with Miniscrew Orthodontics - Case Report

本篇為一右下第二大臼齒舌側傾倒阻生的14歲男性患者，治療方式為以手術移除右下第三大臼齒，同時植入一矯正植體於頰棚區，將皮瓣縫合覆蓋矯正植體及牙齒， 藉以將右下第二大臼齒扶正；此植體矯正之優點為不影響該區肌肉及軟組織運動，得以改善病人治療期之舒適度。針對本病患其他可能治療計畫，舌側傾倒臼齒之治療評估， 矯正植體放置的臨床考量， 以及力學設計等也會在本文中探討。A case of a 14 y/o male patient with a lingually impacted mandibular right 2nd molar was presented. After the third molar was extracted, a closed coil spring was used to connect the impaction to a miniscrew inserted in the nearby buccal shelf. Throughout the treatment process, all orthodontic appliances were hidden underneath the flap. The advantage of this closing flap technique is that it improves patient comfort while minimizing irritation to the surround muscle and soft tissue. Treatment options for uprighting lingually impacted mandibular 2nd molar, clinical considerations of orthodontic miniscrew placement, and design of biomechanics were also discussed.
