Purpose
To present dual-energy CT (DECT) as a reliable diagnostic tool in gout demonstrated by selected clinical cases.
Findings
Previous studies about DECT and gout are reviewed. In addition, clinical cases examined by DECT were presented. DECT scans were performed using a second-generation 128-slice dual source CT system. By using a material labeling algorithm, data were analyzed, visualized and differentiated by assigning different colors to the respective tissues.
DECT has been shown to be highly useful for detecting gouty tophi as well as in preclinical stages, thus allowing early and reliable diagnosis. DECT revealed a relationship between monosodium urate (MSU) deposition and bone erosion. Moreover, DECT enabled accurate monitoring of the response of UA lowering therapies avoiding high radiation burden. It facilitates the detection of tophi in periarticular areas, such as tendons and ligaments. Lastly, DECT improves the reliability of the diagnosis of gout in challenging clinical cases, thus facilitating the decision for appropriate treatment.
Conclusion
DECT represents an excellent and non-invasive diagnostic modality facilitating the diagnosis of gout in a highly specific manner.
