Objective: Past studies have shown inconsistent results regarding the risk of gout recurrence in loop diuretics users compared to thiazide users. To clarify this question, we performed a retrospective cohort study utilizing the National Health Insurance Research Database (NHIRD).
Methods: A total of 15,896 new loop diuretics users and 10,901 thiazide users were matched for age, sex, and entry date. The recurrence of gout after 1 month, 2 months, 3 months, and 4 months of diuretics use was recorded. Relative risk (RR) and corresponding 95% confidence intervals (95% CIs) were calculated.
Results: After 1 month of diuretics use, loop diuretics users had a higher risk of gout recurrence compared to thiazide users (RR: 1.12; 95% CI=1.09-1.14, p<0.001). Consistent trends of increased gout recurrence in loop diuretics users compared to thiazide users were noted after 2 months (RR: 1.06; 95% CI=1.04-1.09, p<0.001), 3 months (RR: 1.07; 95% CI=1.04-1.10, p<0.001), and 4 months (RR: 1.06; 95% CI=1.03-1.09, p<0.001) of treatment.
Conclusion: This study demonstrates that loop diuretics users have a higher risk of gout recurrence compared to thiazide users. Physician need to consider the possibility of gout recurrence when choosing diuretics prescription. |
