Objective: This study aims to investigate the relationship between plasma levels of adipocytokines [adiponectin and retinol-binding protein 4 (RPB4)] and uric acid in patients with primary gouty arthritis (GA).
Methods: 88 primary GA patients and 80 healthy controls (HCs) were enrolled. Clinical data were collected, fasting blood samples were obtained for biochemical examinations such as plasma adiponectin and RBP4 levels, and urine samples were taken for measurement of RBP4 concentrations.
Results: In comparison with HCs, the levels of plasma adiponectin (6.52 ± 6.31 vs 8.46 ± 6.17 μg/ml, P=0.046) were significantly lower, but plasma RBP4 (189.34 ± 126.76 vs 44.17 ± 14.57 ng/ml, P<0.001) and urinary RBP4 (2.44 ± 1.50 vs 0.38 ± 0.16 ng/ml, P<0.001) were markedly higher in GA patients. In GA patients with renal calculus, urinary RBP4 concentrations were even higher compared to those without renal calculus (2.76 ± 1.10 vs 1.30 ± 0.54 ng/ml, P<0.001). In GA patients, uric acid levels were negatively correlated with plasma adiponectin levels (r=-0.36, P<0.001), but positively correlated with plasma RBP4 (r=0.33, P<0.001).
Conclusion: Adiponectin and RBP4 might be involved in the regulation of plasma uric acid and lipid metabolism in GA patients, thus contributing to the pathogenesis of gouty inflammation. |
