The implementation of information technology in home healthcare services aims to achieve coordinated care between hospital-based and home-based medical services, ultimately fulfilling the goal of continuous, holistic care. The implementation period was from 2015 to 2023. The existing issues include: 1) incomplete transmission of medical information, which leads to limitations in home healthcare; 2) the need to optimize the home healthcare information system; and 3) complex administrative procedures that affect the efficiency of home healthcare services. The improvements achieved are: 1) enabling remote access to patient medical records from home, enhancing the completeness and timeliness of medical information; 2) increasing the efficiency of home healthcare nurses while reducing their physical burden by 4 kg; and 3) reducing billing error rates by 50%. This solution ensures continuity of care after patient discharge, and when a return to the hospital is needed, physicians can access home healthcare records, promoting patient safety. This patient-centered approach addresses healthcare provider needs, providing more comprehensive and seamless medical services. |
