本研究旨在探討醫療機構推動數位同意書對環境(Environmental)、社會(Social)及治理(Governance)(ESG)三大面向的具體影響,並深入分析組織文化轉型過程中所面臨的挑戰。隨著醫療數位化趨勢加速,以及傳統紙本作業模式的侷限日益明顯,數位同意書已成為推動電子病歷與永續發展的關鍵措施。本院醫療資訊團隊偕同各醫療科部啟案,全力發展及推動數位同意書系統,使其列入本院數位轉型策略中的關鍵一環;並持續落實在影像檢查同意書、住院手術同意書等無紙化作業;2023年減少紙張用量26,503張,碳排放減少量824 kg CO2e;2024年減少紙張用量87,261張,換算碳排減少1,947 kg CO2e,成效顯著。有效降低紙張使用,提升作業效率,並優化病人就醫體驗。數位簽署流程亦促進醫病間的信任,強化法規遵循與風險管理。然而,在推動過程中,仍面臨如紙本依賴習慣、系統初期操作適應問題,以及部門間支持度不一等挑戰,需仰賴持續的教育訓練、跨部門溝通與政策支持,逐步達成組織整體的數位文化轉型。

This study investigated the specific impacts of implementing digital informed consent system in healthcare institutions across the three core dimensions of Environmental, Social, and Governance (ESG), and further analyzed the challenges encountered during the process of organizational cultural transformation. As healthcare digitalization continues to accelerate and the limitations of traditional paper-based workflows become increasingly apparent, digital informed consent has emerged as a critical driver of electronic medical record advancement and sustainable development. At our institution, the Health Information Technology team, in collaboration with multiple clinical departments, initiated and actively promoted the development of a comprehensive digital informed consent system, positioning it as a key component of the hospital’s digital transformation strategy. The system has been progressively deployed in areas such as imaging examination consent forms and inpatient surgical consent forms, enabling the transition toward paperless operations. In 2023, paper consumption was reduced by 26,503 sheets, corresponding to a carbon emission reduction of 824 kg CO₂e. In 2024, the reduction further expanded to 87,261 sheets, equivalent to a decrease of 1,947 kg CO₂e, demonstrating substantial environmental benefits. Beyond environmental gains, digitalization has effectively lowered paper usage, improved workflow efficiency, and enhanced the overall patient experience. The secure and transparent digital signing process has also strengthened trust between patients and healthcare providers, reinforced regulatory compliance, and supported more robust risk management practices. Nevertheless, the implementation process was not without challenges. These included entrenched reliance on paper-based habits, adaptation difficulties during the early phases of system adoption, and varying degrees of departmental engagement. Addressing these issues requires sustained training efforts, strengthened cross-departmental communication, and supportive institutional policies to gradually achieve a comprehensive digital cultural transformation.
