糖尿病患者控制不良接受針灸治療引發臀大肌膿瘍和菌血症－個案報告

The Development of Gluteal Abscess and Bacteremia in a Poorly Controlled Diabetic Patient Receiving Acupuncture Treatment: Case Report

針灸是傳統醫學的技術，雖然針灸的針十分細小，但針灸仍會有血腫等併發症。然而大範圍膿瘍卻屬罕見。這份報告報導病人接受針灸治療後，發生右臀疼痛與腫脹的主訴，檢查評估病人為臀大肌膿瘍。這位病人在會診骨科醫師後，在病人同意下接受了清瘡手術。術後病人發燒與因疼痛不良於行的症狀緩解。術後順利出院並安排回診追蹤。此報告提醒臨床醫師不論在執行何種微小的侵入性技術，細菌與相關微生物都有可能藉針灸等進入人體，特別要小心免疫不全與衛生概念較不了解的病人。

Acupuncture is a traditional Chinese medicine technique. Despite the short depth and very thin stab, complications-including hematoma-commonly occur. However, large-scale abscesses have rarely been reported. Herein, we report the case of a patient who experienced right hip swelling and pain after undergoing acupuncture. The tentative diagnosis was gluteal abscess formation. The patient consulted an orthopedic surgeon who successfully administered surgery. Regardless of the size of the stab wound, an acupuncture needle increases the risk of inoculating epidermal microorganisms such as Staphylococcus aureus into the body, especially in immunocompromised patients with poor health-seeking behaviors.
