This article explores the experience of Xingyi Clinic, highlighting its service model, target patient groups, diverse care approaches, and the challenges and breakthroughs encountered in its implementation. Xingyi Clinic, emphasizing 3C2A core value framework, ensures seamless care transitions, integrates community resources, and delivers continuous, comprehensive, coordinated, accessible, and accountable care for older adults with disabilities. This approach aims to enhance the quality of life and dignity of elders while reducing unnecessary hospitalizations and emergency visits. In addition, Xingyi clinic is devoted to community development in Taipei City, seeking a transformative solution of rapidly aging society, and to an evidence-based foundation for the development of home-based medical care in Taiwan. |
