Patients with chronic kidney disease often require hospitalization due to multiple comorbidities. Without adequate knowledge of dietary and disease care, dialysis alone cannot prevent potentially life-threatening complications. A recent survey reported a 67% accuracy rate in patient-care knowledge and a 57% satisfaction rate with traditional leaflets or orally delivered guidance. The survey findings revealed a lack of comprehensive education, limited use of varied instructional tools, and insufficient frequency and content in in-service training for nursing staff. This project aimed to improve the accuracy of chronic kidney disease care knowledge through interdisciplinary teamwork by employing diverse guidance tools and organizing relevant educational courses. After implementation, knowledge accuracy rate increased to 95%, satisfaction rate of educational interventions increased to 92%, and the accuracy rate of self-care tracking post-discharge increased to 91%. These results indicate that interdisciplinary strategies effectively enhance the care knowledge and self-care capabilities of patients with chronic kidney disease. |
