This article describes the nursing experience of a 43-year-old male undergoing rehabilitation following his first stroke. The nursing period was from November 2, 2023, to November 20, 2023. The Gordon's 11 Functional Health Patterns to data was collected through physical assessments, interviews, observations, and medical record reviews, including impaired physical activity, self-care deficit, and Powerlessness. During the care period, a strong nurse-patient relationship was established, fostering active listening and encouragement for the patient to express his emotions, thus reducing negative feelings. Collaborating with the medical team, a rehabilitation treatment plan was devised, incorporating holistic and individualized nursing interventions. Professional knowledge and skills were applied to motivate the patient towards rehabilitation, aiding in the restoration of limb strength to maximum functionality and enhancing future self-care abilities, thus improving overall quality of life. Positive support from family members and caregivers increased the patient's compliance and confidence in rehabilitation therapy, leading to successful physical and psychological adaptation and a return to normal life, with a positive outlook towards the future. It is hoped that sharing this nursing experience will serve as a reference for clinical healthcare professionals in caring for similar cases, contributing to enhanced nursing quality. |
