Investigation of a Norovirus Gastroenteritis Cluster in a Hotel, Taoyuan, December 2019

2019年12月12日疾病管制署接獲臺北市政府衛生局通報一起疑似食品中毒案件，初步調查為日本兩高校陸續有多名師生出現腸胃道不適症狀，兩校行程與桃園某飯店有關。為釐清致病原因與傳播途徑，本調查針對發病就醫之甲高校師生進行描述性分析，並聯繫日本感染症研究所衛生調查訓練班，以取得乙高校師生返日後之發病人數及檢驗結果。該飯店工作人員及兩高校學生人體檢體皆檢出諾羅病毒，且兩校發病學生之流行曲線圖皆呈單一波峰，研判為一起食品中毒事件。推測可能因餐廳工作人員休息區洗手設備不完善且清潔度不佳，及部分人員可能洗手不確實，導致餐點或環境受汙染。故建議飯店應加強工作人員教育訓練，落實手部衛生，並改善廚工休息區洗手設備，落實工作環境之清潔與消毒。

On December 12, 2019, the Taiwan Centers for Disease Control received a report from the Taipei City Department of Health of a suspected food poisoning cluster. The preliminary investigation revealed that several teachers and students visiting from two schools in Japan had symptoms of gastrointestinal discomfort. The incident was likely related to a restaurant in Taoyuan. In order to clarify the cause and transmission route of the cluster, a descriptive analysis was carried out on the teachers and students of School A who went to the hospital for treatment. The information from School B was obtained through the Field Epidemiology Training Program of the National Institute of Infectious Diseases in Japan. Norovirus was detected in specimens from hotel staff and students of the two schools. The epidemic curve of the sick students from the two schools showed a single peak, indicating a food poisoning incident. It was speculated that the cluster might be associated with poor hand-washing and improper cleanliness in the kitchen staff's rest area, resulting in contamination of the meal or the environment. We recommended that restaurants strengthen staff education and training, implement strict hand hygiene, improve hand washing equipment in the rest area used by kitchen workers, and implement cleaning and disinfection of the working environment.
