Large Yolk Sac Tumor with Empyema Thoracis in a Young Adult: A Case Report

惡性前縱膈腔卵黃囊腫瘤併發膿胸非常罕見。我們分享一位病例的成功治療經驗。此為一年輕男性患者，因發燒及咳嗽三天就醫，經一系列檢查及腫瘤切片後而確診上述疾病，患者同時併有肋膜腔及肺臟轉移。因右側膿胸，患者先接受胸腔鏡手術及抗生素治療控制感染，再接受後續化學治療以及局部放射治療。最後施行腫瘤切除術時，病理報告證實已沒有殘留的惡性細胞。該患者在後續18個月的追蹤中都沒有復發。

Cases of large mediastinal yolk sac tumors with empyema thoracis are rare. Herein, we report a case of a young male patient with a large anterior mediastinal yolk sac tumor presenting with fever and a cough that lasted three days. The yolk sac tumor was confirmed through a tumor biopsy, indicating pleural seeding and pulmonary metastasis associated with right-sided empyema thoracis. The patient first underwent thoracoscopic decortication and was administered empirical antibiotics before receiving chemoradiotherapy. We observed no residual malignancy at the final excision of the remaining tumor. The patient had no yolk sac tumor recurrence at the 18-month follow-up.
