Chemical drug extravasation can be fatal. The prevention of chemical drug extravasation is a patient safety goal. In 2016-2017, the extravasation rate was 0.04%. A committee was formed to investigate causes of extravasation and convened from January 1 to December 31, 2018. The findings of the committee led to a project aimed at making improvements that included 1) the formation of a chemotherapy extravasation disciplinary team to promote the use of port-a-cath (port) and a peripherally inserted central catheter (PICC) and 2) the development of a chemotherapy catheter care and chemotherapy administrative process review document and a process for tracking chemotherapy extravasation. The extravasation rate decreased from 0.04% in 2017 to 0.02% in 2020. This project achieved its goal of improving the quality of patient care and better ensuring the safety of chemotherapy patients. |
