The role of platinum or taxanes chemotherapy during the second and third trimester in cervical cancer

子宮頸癌乃懷孕中最常見的婦科癌症，尚無標準的治療共識，需視母親和胎兒的健康而異，故需整個相關團隊人員，以有計畫共識性的治療。分析子宮頸癌在懷孕時預後不錯，因此建議懷孕中處理乃保持胎兒為優先考量。對第Ib1期子宮頸癌，宜延遲至胎兒成熟、生產後再行治療，對晚期子宮頸癌，宜於第2、3孕期予platinum或taxanes為前導性化學治療，對母子均佳。

The treatment of cervical cancer during pregnancy mirrors that outside of pregnancy, with a balance between maternal versus fetal health. Cervical cancer is the most common gynecological cancer encountered in pregnancy. There are no standardized procedures concerning the treatment of cervical cancer in pregnancy. Therefore, in consultation with the patient and a multidisciplinary team, an adequate individualized treatment plan should be determined. Analysis of prognosis of cervical cancer during pregnancy shows no negative impact of pregnancy on the outcome of patients; therefore, pregnancy-preserving management should be considered initially. Delayed treatment can be offered for stage Ib1 cervical cancer. Neoadjuvant with platinum or taxanes chemotherapy can be given for advanced cervical cancer during the second and third trimester, in which could be a good disease-free survival without significant adverse neonatal outcomes.
