一位肝硬化併發自發性細菌性腹膜炎患者之護理經驗

A Nursing Experience for a Patient with Peritonitis Secondary to Liver Cirrhosis

本文描述一位肝硬化腹水併發自發性細菌性腹膜炎個案，入住加護病房的護理經驗，諏理期間為2008年11月26日至2008年12月8日。筆者運用Gordon 11項功能性健康評估，藉由觀察、會談等方式收集資料，確認個案主要的健康問題有「低效性呼吸型態」、「睡眠型態紊亂」及「潛在危險性傷害」等護理問題。護理期間運用傾聽、溝通、衛教等技巧，透過直接的護理活動，適時教導個案減輕合併症帶來的不適，使個案學習自我照護，協助個案度過生理上的危機，照護後個案病情穩定，順利轉出加護病房，禆益於出院前準備。

This article describes a nursing experience for a patient suffering from spontaneous becterial peritonitis, a comlication of liver cirrhosis in a medical intensive care unit. The nursing period was from November 26 to December 8, 2008. The authors collected data through patient observation, patient interview, and physical assessment with Gordon's 11 Functional Health Assessment Tool. The three major identified health problems were inffective breathing patterns, potential risk for injury, and sleep disturbance. With the utilization of active listening and communication skills, the author taught the patient self-care to relieve discomfort due to the complication. The cuthor assisted the patient in going through the critical stage of physiological crisis. The patient was transferred out of the intensive care unit and was helped with descharged once conditions stabilized.
