建置護理品質監測系統－－以身體約束指標為例

Establishing and Evaluating the Nursing Informatics System for Quality Indicators Monitor

醫護品質的提升是病患安全的保障，然而建立醫護提供者接受度高的資訊系統才能作為持續改善品質的依據。本研究的目的為設計與建置護理品質監測資訊系統；並評價護理人員對身體約束品管指標監測資訊化之接受度。研究分為二部分，第一部份是設計與建置中部某醫學中心之護理品質監測資訊系統，流程包括環境分析及確認需求、系統及資料庫分析與規劃、系統開發、系統測試、全面推廣等5個階段，以螺旋方式反覆地重新修訂系統，使其達到最符合使用者需求。第二部份是評價資訊系統之成效：護理人員對身體約束品管指標監測資訊化之接受度及花費時間，於系統建軍完畢全院實施八週後以問卷測其接受度。結果顯示在系統功能需求包括監測指標資料的新增、修改、刪除、查詢、自動化的檢查資料、即時上網查詢統計報表及圖表、檢討改進分析及自動傳送E-mail到所屬單位或人員等。在身體約束指標監測資訊化復之接受度，共有419位護理人員參與，接受度95%以上，不接受之項目比例較多者為「身體約束監測電腦作業系統電腦運作之速度」4.5%。花費時間方面，手寫資料輸入較電腦輸入多花費0.49分鐘，然每月分析及檢討報告則節省了46.5分鐘。此護理品質監測資訊系統之建置可增加品管資料收集之正確性與完整性，更具有管理及流程簡化的加佳作用，是護理品質監測重要的改造工程，可提供其他護理品質監測資訊化的基礎。

The purposes of this research were to design and build up a nursing quality surveillance informationsystem for appraising care workers' acceptability. The research was divided into 2 parts.The first part contained the designing and building up of nursing quality information system. Thesecond part involved appraising care workers'acceptability for corporal repression quality targetsurveillance information processing. The results that appeared in the system function included examinformation automatically, statistics, graphs, analysis and E-mail automatically. As for the acceptabilityafter corporal repression surveillance information processing, 419 nurses joined in this programandthe acceptability was over 95 percent. The systemcan increase the correction and completionof quality controller's data collection, more benefits and simplify the work flow.
