照顧一位以清潔劑自殺之青少年個案的護理經驗

Nursing Experience of a Suicidal Teenager at I.C.U.

本文探討一位以清潔劑「威靈頓」自殺之青少年於手術後轉入加護病房之護理經驗。對於此自殺個案的照顧，除了急性期的緊急處置之外，再以羅氏適應模式評估，確認個案之健康問題為：無法有效維持呼吸道通暢、現存性損傷、個人因應能力失調、身體心像紊亂、潛在危險性感染及舒適情況改變。經由良好護病關係的建立，以及照顧個案的醫護人員們一致的關懷態度，讓個案由完全不理睬家屬及醫護人員、拒絕與外界溝通，轉變成將護理人員視為諮詢對象，使護理人員得以協助此個案面對身體、心理適應問題的經驗，與大家分享。

This article is to discuss the nursing experience of an adolescent who attempted suicide with detergent 、ilington?and was transferred into the intensive care unit. In addition to stabilizing the patient、 physical imbalance induced by chemical substance, by Roy、 Adaptation Model, the patient’s health problems were identified as: ineffective airway clearance、existing injury、ineffective individual coping、body image disturbance、risk of infection and altered comfort. Based on the therapeutic relationship between the patient and nursing staff, the patient turned from rejecting the communication with the nursing staff into accepting the nursing staff as consultants. The nursing staff could further aid the patient in facing with the problem of physical and psychological adaptation. We present the caring experience to be shared with all the nursing staff.
