To implement competency-based medical education (CBME) in daily medical practice, we have, since 2016, established workplace training for radiation therapists based on the framework of entrustable professional activities (EPAs) and a system for assessing them within the same framework. These efforts have included the following: formulation of profession-specific EPAs through consensus methods, validation of EPA-based workplace direct observation and assessment tools, rater training to promote interrater reliability, and formation of a clinical competence committee. We designed our training experiences and assessment systems on the basis of the EPAs framework. Furthermore, we developed EPA-based workplace direct observation and assessment tools to build preceptors’ consensus on clinical training and optimize information gathering through multiple assessment strategies. Through these efforts, the validity of workplace observation and reliability of summative evaluation were improved and core competencies were integrated into daily observable medical practices.
