接受抗凝血藥物病患併發腎臟腎血管肌肉脂肪瘤出血：病例報告

Renal Angiomyolipoma Complicated with Perirenal Hemorrhage in A 60-year-old Anticoagulated Patient: A Case Report

患者是一位60 歲女性，服用warfarin 四天後右腰突然疼痛至本院急診求診。腹部電腦斷層攝影發現右腎有一腫瘤，且在腎臟周圍有血腫現象。因為病患生命徵象穩定，在右腎血管肌肉脂肪瘤自發性破裂診斷下，停止使用warfarin 並以保守療法治療，病患並沒有接受手術治療，出院後追蹤檢查康復情形良好。 自發性腎臟出血相當少見，老年病患接受抗凝血藥物治療時應該特別注意是否有併發症的發生。

A case of spontaneous perirenal hemorrhage in an anticoagulated patient is reported. Spontaneous perirenal hemorrhage is uncommon and is seldom suspected clinically. Elderly subjects are exposed to a higher risk of bleeding complications during warfarin treatment. A 60-year-old female experienced acute right flank pain after receiving warfarin for 4 days. Detailed imaging modalities demonstrated the presence of perirenal hematoma and angiomyolipoma. The patient was treated conservatively and followed up with ultrasonographic and computed tomographic scans until the hematoma was almost completely reabsorbed. No surgical intervention was needed, and the patient recovered well.
