http://www.gouthyperuricemia.com Objective: Impotence is a disease with multifactorial etiology, which includes psychiatric disorder, diabetes, obesity,alcohol use as well as inflammation. Gout disease is also comorbiditied with the aforementioned risk factors, thus we intended to examine the association independently between incidence of impotence and gout. Methods: Two designs, a retrospective cohort and a nested case-control, were used to reveal the study purpose by using a national outpatient data base of one million random subjects and covering years 1997 to 2008. The controls were selected by matching the case age groups on a ratio of 5:1. Other diseases were explored as risks for impotence, such as hypertension,diabetes, hyperlipidemia, chronic renal failure and depression. Standardized incidence ratio (SIR) and logistic regression were applied to reveal the statistical significance. Results: A total of 6,212 gout and 31,060 controls were included in the retrospective cohort design, while the nested case-control design included 2,631 cases of impotence and 13,155 controls. The incidence of impotence per 1000 person-years was 3.30 cases among gout patients compared to 1.79 cases among controls (SIR=1.86, 95% CI=1.57-2.20).Depression, DM, hyperlipidemia, gout and hypertension have significant association with impotence in the nested case-control study (adjusted ORs were 2.13, 1.45, 1.31, 1.19 and 1.13, respectively; all p<0.05). Conclusion: Gout disease contributes independently to the occurrence of impotence explored by both retrospective cohort and nested case-control designs after adjustment of depression, diabetes, hyperlipidemia, renal failure and hypertension.
