卵巢混合性生殖細胞腫瘤之復發及惡性轉形：病例報告與文獻回顧

Recurrent ovarian mixed germ cell tumor with unusual malignant transformation: a case report

卵巢混合性生殖細胞腫瘤（ovarian mixed germ cell tumor）存活率通常良好，然若在重複減積手術及化療後仍復發，預後相對不佳，臨床醫師應審慎評估病人之危險因子，確立個別化的治療方針及追蹤策略；本文報告一卵巢混合性生殖細胞腫瘤病例合併罕見之惡性轉化（malignant transformation）組成，且存有畸胎瘤不成熟成分化學治療反轉化的現象（chemotherapeutic retroconversion），並回顧現有相關文獻。

The value of this report is the identification of late recurrence with an extremelyunusual combination of malignant transformation. In particular, the retroconversionof immature to mature teratoma as well as a somatic-type malignanttransformation were both observed postchemotherapeutically in our case. Recurrentdisease after repeated cytoreduction and chemotherapy hints a poor outcomedespite a generally excellent long-term survival rate among ovarian germcell malignancies. It is important for clinicians to distinguish those at risk ofpoorer outcomes and establish individualized postoperative surveillance.Fertility-compromising surgery may be considered in selected patients.
