誤認為惡性卵巢腫瘤之巨大子宮內膜異位瘤病例討論及相關文獻回顧

A big ovarian endometrioma mimicking an epithelial ovarian cancer: case report and literature review

子宮內膜異位瘤在卵巢的良性腫瘤比例當中佔有一席之地，但巨大至三十公分以上、內容物兩千七百毫升的子宮內膜異位瘤極為罕見，且與卵巢惡性腫瘤在臨床症狀、腫瘤型態、實驗室指標、影像學特性上有許多極易相互混淆之處，且如診斷靠病理組織翻盤有治療方針差異的顧慮，應在著手治療前儘可能增加診斷的準確度，在此提出一個巨大子宮內膜異位瘤的病例，並討論相關的定義、盛行率、各項診斷依據、治療等文獻回顧。

Endometrioma is relative common among benign gynecological tumor. However, it is rare to encounter huge mass up to more than 30 centimeter, contents up to more than 2700ml turn out to be benign endoetrioma relative than malignancy tumor. In addition, there are many similar aspect including clinical symptoms and signs, tumor morphology, labotory data and image among ovarian neoplasm and endometrioma. Furthermore, under the circumstance of changing the diagnosis after pathology proven, the way of operative plan may change too. It is no doubt that accurate diagnosis before operation and pathology result give much more benefit to our patients. Hence, we present one case of extremely huge ovarian mass turn out to be endometrioma, in order to raise the preoperative diagnostic rate. At the same time, we review the related endometrioma definition, prevalence, diagnostic criteria and tricky point, treatment and literature review.
