This article described the nursing experience of caring a burned senior patient with dementia. The author was the primary care nurse of the patient from August 27th to October 17th in 2011. Through the review, discussion, practice, and teamwork over this care course, two nursing problems were identified, which were (1) poor communication, lack of effective strategies and skills in communication with dementia patients and (2) the obstacle of discharge preparation, lack of primary caregivers. We developed effective methods to resolve these two problems. The first is to enhance the communicating skills with patients, especially in dementia ones. Furthermore, with the assistance from the medical team, the patient was smoothly discharged from the hospital and placed in the elder care facility. We demonstrated the importance of the teamwork and longitudinal care on caring for elderly patients. We hope that the special experience could be an important reference for other health care workers. |
