本文描述一位中度智能障礙青少年初次診斷第一型糖尿病之護理經驗,因個案中度智能障礙,加上案母對於照顧個案缺乏信心,故引發筆者動機,照護期間為2016年6月6日至6月21日,住院過程,運用整體性護理評估,確立個案主要健康問題有不穩定血糖濃度、無效性健康維護能力及案母照顧者角色緊張等健康問題。照護過程,藉由配合個案的心智年齡,使用對話式閱讀技巧及個別化繪本,衛教個案糖尿病相關自我照顧技能;提供案母舒緩獨立照顧個案壓力之方法;使個案血糖穩定出院,提升案母照顧個案信心,並持續提供返家後續照護計畫,維持個案糖尿病控制成效。(志為護理,2019; 18:3, 125-136)
In this paper, we report the experience of providing nursing care for a patient with moderate mental retardation who was admitted for type 1 diabetes treatment. Because the patient had moderate mental retardation, the patient's mother, who independently provided care for the patient and had limited knowledge of care and this disease, was considerably anxious and required help. During hospitalization from June 6th, 2016, to June 21st, 2016, the nursing staff performed a full care evaluation and found that the patient had an unstable blood glucose level and received ineffective health maintenance and that the caregiver was under considerable stress regarding providing care. During the nursing process, considering the intelligence level of the patient, we used conversational reading skills and individual picture books to provide information regarding diabetes-related self-care skills to the patient. We simultaneously attempted to reduce the mother's care burden. Finally, at the time of discharge, the patient had a stable blood glucose level and the mother was confident in providing care for the patient. After discharge, we continued to provide follow-up care to help the patient maintain a stable condition. (Tzu Chi Nursing Journal, 2019; 18:3, 125-136) |
