血崩昏厥之中醫療法病案報告

Treatment of Metrorrhagia with Syncope using Traditional Chinese Medicine

血崩昏厥為嚴重的婦科急症，中醫古籍雖有治療嚴重血崩的記載，但現代中醫治療血崩昏厥的病例仍不多。本病案提出中醫住院治療一血崩昏厥的15歲少女，於月經連續大量出血不止後，出現嚴重的頭暈、虛弱、胸悶以及心悸感，並兩度因血崩而昏厥，在經過西醫賀爾蒙療法與止血劑治療後仍未改善，故改採中醫順勢療法住院治療，以傅青主雙補脾腎（寬帶湯）與去瘀生新（生化湯）的治療方法，並隨症搭配密集的針刺、拔罐及藥草薰蒸後，出血隨即停止，頭暈、胸悶、心悸也已消失，在住院第3天就已控制嚴重血崩的病情，在前後共5天的治療後出血停止，病情穩定而出院。出院後仍接受中醫門診調理治療及追訪3個月，不僅行經天數由中醫住院前的20餘天減少到正常的7天以內，且血色素也由6.9g/dL上升到10.0g/dL。藉由中醫住院模式可以密集監測病況進展，即時修正治療方向，得以充分發揮中醫治療效果，是治療急重症病患值得探取的治療模式之一。 Massive metrorrhagia with syncope is a gynecological disease that is considered an emergency. Although Traditional Chinese medicine’s (TCM) classical literature describes treatment for massive metrorrhagia, there are still very few clinical trials that use TCM for this disease. Here we describe the case of a 15 year-old teenager who presented intense metrorrhagia with severe dizziness, weakness, chest stuffiness, palpitations, and two episodes of syncope. After five days of TCM hospitalization using Kuan-Dai-Tang and Sheng-Hua-Tang formulas, Fu Qing-Zhu's Gynecology, frequent acupuncture, cupping, and herbal fumigation as treatments, the bleeding ceased and symptoms were relieved. After being discharged, she continued to receive out-patient treatment for three months. During this time her menstrual duration decreased from 20 days to less than seven days, and her hemoglobin increased from 6.9 to 10.0 g/dL. TCM hospitalization therapy can give an intensive treatment and follow up and should be considered a viable and acceptable treatment option for patients who suffer from massive metrorrhagia with syncope.
