優化智慧型檢體追蹤管理系統

Optimization of a smart system for tracking and managing specimen delivery

文獻顯示70％的醫療重要決策取決於檢驗醫學的結果，若延遲檢體傳送會影響醫療處置甚至危及病人生命，且如果發生檢體遺失，更可能引發醫療爭議事件。本專案將護理採集、勤務傳送至檢驗收檢過程，運用智慧化系統功能達到有效的管理及追蹤，降低報告延誤、檢體傳送錯誤，以提升醫療品質病人安全。運用精實思維之目視化管理及銷售點資訊管理系統（Point of sale，POS）概念，建置條碼掃描檢體追蹤資訊系統，針對三個監控端：護理端、勤務端及檢驗端，全程監控檢體傳送過程。優化智慧型檢體追蹤管理系統後包括清單錯誤率：改善前3.88％（777筆）、改善後0％（0筆）；檢體送錯科室件數：改善前268筆、改善後0筆；檢體遺失數：改善前24筆、改善後0筆。另外降低檢體因勤務送錯單位而浪費時間，每月約省2680分鐘；省下大量紙張與列印時所需碳粉及印表機耗損，每年約省31萬張紙耗等附加效益。本專案優化智慧型檢體追蹤管理系統，以系統思維發掘護理人員採血、勤務傳送及檢驗人員檢體時之潛在問題，提供正確安全的檢體準確度，增進工作人員工作效率，防範檢體錯誤及遺失，減少醫療糾紛，達到「病人安全」的照護目標。 The recent literature shows that 70% of important medical decisions are based on the results of laboratory tests. Delayed specimen transport will hinder the efficiency of treatment; even threaten the patient's life. In addition, any loss of specimens is more likely to trigger disputes. This study aimed at optimizing a smart system in which data regarding specimen collection and transport was recorded and tracked, thereby reducing delays in reporting and sample transport errors to improve the quality of patient care. Using the visualized management and point of sale (POS) concept of lean thinking, a barcode scanning and specimen tracking information system was established with three monitoring points: specimen gathered at the nursing station, specimen transport to the laboratory by the servicemen, and comprehensive monitoring of specimens. After optimization of the intelligent sample tracking management system was implemented, the error rate of the sample list decreased from 3.88% (777) to 0% (0); the number of samples sent to the wrong department was reduced from 268 to 0; the number of missing samples was reduced from 24 to 0. In addition, 2,680 minutes were saved per month because of 100% correct delivery to the right destination, and 310,000 pieces of paper with toner and the printer´s expenses were saved per year due to the paperless tracking system. In conclusion, the smart management system eliminated the potential problems in nurses collecting specimens and service personnel specimen transport for laboratory tests. It provides accurate and safe specimen delivery, improves staff productivity, prevents mistakes and loss of specimens, reduces medical disputes, and enhances the ultimate goal of patient safety.
