Methadone maintenance therapy (MMT) has been used for treating opioid addiction by Dole and Nyswander since 1965. Whether the MMT is needed for a lifetime is still unclear. Here, we present a case of a patient who developed detoxication phobia in the planned methadone reduction program. A 33-year-old married woman patient, who graduated from a vocational high school and lived with family, has become dependent on heroin since her age of 25 years. Initially she had smoked heroin for half a year, before she shifted to its injection use. She was a self-employed worker at a factory owned by her wealthy and supportive family. She was not found to have other psychiatric diseases at the initial assessment. The patient was assigned to our clinic for the MMT for the fi rst time when she was 27 years old because of the deferred prosecution program. The initial methadone dose was 30 mg per day, and the maintenance dose was 50 mg per day. After her having participated in the MMT daily under supervision for three years, methadone reduction program was suggested at her family's request. She had also been abstinent from heroin for more than two years. She presented herself with good medical adherence. |
