免疫系統除了能辨識且清除變異細胞以外,同時也在致癌過程中扮演重要角色。免疫系統在細胞癌化病程中所涉及/參與的步驟,包括清除變異細胞、對峙平衡與癌細胞逃脫管道。癌症免疫療法,翻轉了人們對腫瘤生理、病理與治療的認知,同時也發展出更多的武器來對抗腫瘤,包括單株抗體、細胞免疫治療、癌症疫苗以及免疫檢查點抑制劑與T細胞活化(CAR-T療法)等。這些新近發展的治療方式,大幅提升對抗腫瘤及惡性血液疾病的治療成績。
Abstract: Accumulating evidence has demonstrated that the immune system plays a critical role in recognizing, eliminating transformed malignant cells and also in promoting tumor progression. The role of the immune system to suppress and promote cancer growth is termed “cancer immunoediting” and consists of three phases: elimination, equilibrium, and escape. The success of immune checkpoint blockade and understanding of cell-mediated immunity represents a turning point in cancer immunotherapy. The clinical goal of cancer immunotherapy is to provide either passive or active immunity against malignancies by harnessing the immune system to target tumors. Recently, monoclonal antibodies, cellular immunotherapy, vaccines, immune checkpoint therapy, and chimeric antigen receptor T-cell immunotherapy have increasingly become successful therapeutic agents for the treatment of solid and hematological cancers in clinical practice. |
