醫策會於2015年將以病人為焦點之查證方式(Patient Focused Methodology,PFM)導入醫院評鑑。PFM是以病人為焦點的評鑑驗證方法學,藉由系統性地規劃來檢視醫療機構對病人的醫療執行過程與執行成效。個案醫院為臺灣南部某醫學中心,籌備期間依據(1)組織規劃,資源籌備;(2)樹立PFM 模範教案;(3)強調深度與廣度的教育訓練;(4)例行性查核,評鑑日常化;(5)辦理全院性模擬評鑑。個案醫院於2017年順利通過醫院評鑑。醫院可透過PFM規劃病人路徑及系統路徑查證,有系統及策略的協助醫院逐層檢視病人就醫流程與環境安全,並發掘缺失進行改善。本文提出的以PFM推動醫院評鑑籌備之流程與建議可供其他醫院學習仿效。
To improve the quality of medical care, the index hospital introduced apatient-focused methodology (PFM) to review the process and outcome of medical care through systematic strategic planning. PFM is a methodology based on a patient-centered concept, which can help medical personnel to resolve problems comprehensively and thoroughly. This article explores the function and possible applications of PFM. |
