肺痿咳血之中醫診療病例追蹤報告

A Case Report of Treatment with Traditional Chinese Medicine for Fei Wei Hemoptysis

本例患者為62歲女性，長期咳嗽伴見帶有血絲的黃綠色痰，西醫診斷為慢性支氣管擴張（chronic bronchiectasis）合併血小板缺乏症（thrombocytopenia）。自民國73年症狀反覆發作持續28年未能治癒。由於咳嗽帶血絲之症狀一直難以改善，故患者前來本院就診。中醫診斷為痰熱瘀血阻肺，氣血俱虛之症。故採用滋陰清熱，潤肺化痰止咳止血之法治療。經4個月的治療之後，患者咳嗽次數及痰量均有顯著減少，痰色由黃綠色轉淡，血小板由治療前66,000/uL提升至110,000/uL。患者持續門診追蹤治療中，狀況良好。

The patient was a 62 year-old female and diagnosed with chronic brochiectasis and thrombocytopenia. She had been suffering from severe cough and green-yellow sputum with blood since 1984. Failing to successfully treat her severe cough with hemoptysis, she came to our hospital seeking help. The traditional Chinese medicine (TCM) diagnosis was combination of sputum, heat and blood stasis in lung, and deficiency of qi and blood. We used the principle of enrichment of ying with clearing heat, diffusing the lung to resolve phlegm. In addition, inhibition of cough and bleeding were also used. Frequency of cough and amount of sputum has decreased after four months of treatment, and futhermore, platelet count increased from 66,000/uL to 110,000/uL. The patient had continued follow up in our hospital.
