使用矯正迷你骨釘改正單側完全頰側錯咬合併嚴重第二級骨性之病例報告

Severe Skeletal Class II with Unilateral Crossbite Treated with TADs: A Case Report

單側頰側後牙錯咬在矯正治療中是相當複雜及具困難的病例。造成單側或雙側錯咬發生的原因可能來自於齒源性（dental）、骨性（skeletal）或神經肌肉（neuromuscular）等功能性的問題造成。本病例報告為一23歲女性病患，有嚴重第二級骨性關係及合併右側完全後牙頰側錯咬（complete posterior buccal crossbite）問題。藉由矯正迷你骨釘（miniscrew）當骨性錨定（skeletal anchorage）並配合完全覆蓋齒列的咬合板（full coverage biting plate）有效率的改正頰側錯咬問題。在病患不願接受正顎手術的情況下，仍以單純矯正治療達到良好的垂直方向控制以及減少了極大的水平覆咬問題。

Unilateral posterior crossbite case is quite complicated and difficult for orthodontist. The etiology may include any combination of dental, skeletal and neuromuscular functional components. This case is a 23 years old female with severe class II skeletal pattern and right side posterior teeth complete buccal crossbite. With the aid of TADs (temporary anchorage device) and full coverage biting plate, unilateral posterior buccal crossbite was corrected in an efficient way. Without an orthognathic surgery, the results of good dentition control in vertical direction and large amount of overjet reduction can still be obtained.
