一位急性心肌梗塞接受冠狀動脈繞道手術病患之加護護理經驗

Nursing Experience in Intensive Care Unit for a Patient with Acute Myocardial Infarction Undergoing Coronary Artery Bypass Surgery

本文描述一位急性心肌梗塞接受冠狀動脈繞道手術病患之加護護理經驗。照護期間自2015 年1 月19 日至2015 年1 月29 日，筆者藉由觀察、會談、身體評估及病歷查閱等方式收集資料，並運用Gordon 十一項功能性健康型態評估確立病患主要之健康問題有：心輸出量減少、急性疼痛及焦慮。筆者依病患之個別性、配合醫療處置，維持病患血液動力學穩定；透過主動關懷、教導疼痛控制的方法及提供手術前後相關照護資訊，緩解病患急性疼痛及焦慮。期盼此護理經驗能提供護理同仁臨床照顧參考。

The purpose of this article was to explore a nursing experience in intensive care unit for a patientwith acute myocardial infarction undergoing coronary artery bypass surgery. From January 19 to January29, 2015, data were collected through observation, interview, physical examination, and chart reviewaccording to Gordon’s 11 functional health patterns. The health problems identified included decreasedcardiac output, acute pain, and anxiety. Through individual medical measures, the author helped thepatient to maintain hemodynamic stability. By providing active care, method of pain control, and surgeryrelatedinformation, the author relieved the patient’s acute pain and anxiety. This nursing experience canserve as a reference for the care of such patients.
