A 71-year-old divorced male patient with schizophrenia had his fi rst psychotic episode at age of 32 years. He had never had a manic episode or depressive episode previously. He was hospitalized twice due to having psychotic symptoms during his age of 40's. He was stabilized with sulpiride 400 mg/day, and received follow-up at our outpatient clinic. At age of 65 years, he was admitted again owing to having disturbances of consciousness, unstable gaits with frequent falls, and prominent manic symptoms (including elated mood, psychomotor agitation, grandiose delusion, decreased need for sleep, and disruptive behaviors). During admission, he received psychological tests. The result of his Wechsler Adult Intelligence Scale - III (WAIS-III) showed verbal intelligence quotient (VIQ) = 101, performance intelligence quotient (PIQ) = 85, full intelligence quotient (FIQ) = 94. The result of Mini-Mental State Examination (MMSE) showed a total score of 24/30. He was rated 32/56 using Young Mania Rating Scale (YMRS), implying having a moderate severity mania during that period. The fi nding of Bender Gestalt Test showed a total score of 84, not implying having an organic brain damage. Past medical history included ischemic heart disease and hyperlipidemia. |
