This is a case report of nursing care on a woman suffered from perinatal loss. The pregnancy was terminated at 22nd week due to fetal abnormality. The case was under inpatient care for 4 days and was followed up with monthly phone calls for 3 months after hospital discharge. Roy's adaptation model was used during the entire care process. Information was collected through various techniques such as observing, interviewing, consoling, listening, and reviewing medical record. Our nursing care was to identify problems and to promote physical and mental adjustment. The major problems identified were pain, grief and disrupting sleeping pattern. Patient centered care was focused on maintaining physical comfort for the patient and helping the patient and her family work through the grief process by providing emotional support or positive feedback.
