類似類風濕性關節炎之多中心性網狀組織細胞增多症－－病例報告

Multicentric reticulohistiocytosis mimicking rheumatoid arthritis in a 35-year-old woman – case report

一位35歲女性主訴兩個月嚴重搔癢之閃亮皮疹合併一個月類似類風濕性關節炎多之發性關節炎。但是所有實驗室自體免疫抗體，補體檢查以及腫瘤標記篩選都是正常。關節液抽取檢查顯示白血球略為增加，其中多數為淋巴球。而皮膚切片病理報告證實為多中心性網狀組織細胞增多症。該病患接受兩個月之非類固醇消炎藥，奎寧，salfasalazine，methotrexate治療，對皮膚病變以及關節症狀效果不佳。本篇論文報告此例少見病例。

A 35-year-old lady initially presented with two-month history of severe pruritic shining skin lesions couple with one-month history of polyarthritis involving the joints similar to the patterns of rheumatoid arthritis. However, the laboratory autoantibodies, complements, and tumor markers investigation disclosed non-significance. The joint fluid study showed mild increased cell counts with lymphocyte predominant. The skin biopsy confirmed to be a case of multicentric reticulohistiocytosis. Non-steroid anti-inflammatory drugs, hydroxychloroquine, sulfasalazine and methotrexate were administered for two months, but were ineffective. We report this case for its rarity.
