The key issue of anesthesia is to block nociceptive input to reduce surgical stress and to preserve physiological homeostasis. With cumulative knowledge in peri-operative care, anesthesiologists are enabled to extend their involvement in intensive care, resuscitation and acute pain service. Thus, the principal role of an anesthesiologist is to avoid the occurrence of altered homeostasis that may lead to regulatory maladaptation and decompensation primarily through sympathetic activation. As renal blood flow is predominantly regulated by sympathetic nerve, the anesthetic-induced disturbance of sympathetic nervous system can jeopardize renal perfusion and thereby leads to profound renal damage or acute tubular necrosis. |
