子宮肌瘤的另類藥物治療

Alternative therapy for uterine myomas

子宮肌瘤是一種良性，荷爾蒙依賴性的平滑肌瘤，傳統上除了手術切除乾淨外，內科或藥物的治療比較少被討論。在目前用來治療子宮肌瘤的策略，一般仍以阻斷內生生性的女性賀爾蒙為主，在有限的藥物治療中，以性腺釋放激素的刺激劑，leuprolide，被公認是最有效的藥物之一，只可惜其導致的提早或暫時性更年期的狀態，不管在婦女健康或身心不適部分，都非長久使用之策。最近的前瞻性研究發現選擇性的黃體素受器調節劑，例如ulipristal，可能也有治療子宮肌瘤的效果。本文就簡介這些報導。

Uterine myomas (called 'myomas', 'fibroids') are the most common hormone-dependent benign tumor in the female reproductive tract. Conventionally, surgical intervention is the most popular and definite treatment for symptomatic uterine myomas. Medical treatment is rarely discussed. Gonadotropin releasing hormone agonist (GnRH agonist) is one of the best drugs in the management of symptomatic uterine myomas; however, since this drug induces transient menopausal status of young women, who might bother from both physical and emotional health problems, the use of GnRH agonist is often temporary. Recently, selective progesterone receptor modulator has been reported to be effective in the management of uterine myomas. The aim of this brief report tried to introduce recent advances in medical treatment for uterine myomas.
