一位接受手部整形手術之幼兒的護理經驗

Experience of Nursing a Toddler Receiving Toe to Hand Surgery

住院手術對兒科病童而言，會產生害怕及不安的情緒，因醫療環境陌生且其對醫院中各項常規不瞭解。本篇個案報告為照護一位幹受手部整形手術（toe to hand）幼兒的護理經驗。護理過程是以非結構性會談、觀察收集相關資料。歸納出健康問題包括害怕、疼痛及身體活動功能障礙。在照護過程中，運用兒科溝通技巧及遊戲，建立個任感與護病關係，協助病童表達內心害怕的事情，讓護理人員及早瞭解病童的問題以提供協助，文中提出的臨床經驗，期望能做為日後照護類似個案的護理人員參考。

The purpose of this study is to examine the experience of nursing a 3-year-old boy receiving toe to hand surgery. Nursing assessment was based on the framework of Gordon's functional evaluation. Data was collected by participant observation and interview. Three health problems, including fear, pain and impaired physical mobility, were found. During the period of care, play and communication skills were used appropriately. It helped the patient to express the fear in his mind. This nursing experience can help nurses to earlier understand the problems of similar cases. It also can help nurses to build up trust and the nurse-patient relationship. The nursing experience from this case study can help nurses who will take care of similar cases, and thereby promote quality of nursing care and provide continuity of care.
