遠距安寧照護創新服務模式與系統之研究－－居家訪視師介面

A Pilot research on an innovation telehospice service model and system--for Home visiting nurse

安寧照護是提供末期病人完整隻身、心、靈的安寧照顧，積極緩解其身體及心理等不適，而以科技協助解決末期病人及家屬在選擇居家安寧照護服務時所面臨的困境，將為未來趨勢。因此本研究與醫護團隊合作，首要為結合個案管理並設計一合適的遠距安寧照護服務流程，以此為基礎，再透過資訊網路，規劃一個遠距安寧照護平台，提供遠距安寧照護之醫師與個案管理師使用，此計畫中經由遠距安寧照護服務模式與流程確立、系統評估與規劃、系統芻型開發與建置、系統芻型整合與測試與臨床實驗等研究步驟。此平台主要為協助末期的病患，在病情穩定且不需住院治療的情況下出院回家後，仍然能持續獲得醫護團隊持續性的照護。

Hospice care provides a complete care for terminal patients to ease their physical and psychological discomfort. And the technology will improve the telehospice service at home in the future. The team of this study is an interdisciplinary cooperation. We combine case management and design an appropriate telehospice service processes and platform for home visiting nurses and physicians. The study will set a telehspice service model, service processes, evaluation, system and clinical plan. The platform will provide enough support networks in the skills and knowledge of care for terminal cancer patient to get the continuous care by the hospice care team of the hospital when the patients stay at their homes.
