強迫症在單次電氣痙攣治療後症狀急速改善

Dramatic Improvement of Obsessive-compulsive Symptoms after a Single Electroconvulsive Therapy

背景：電氣痙攣治療在精神科治療中，經常是當做最後一線的治療，但很多患有重大精 神疾病，精神症狀嚴重，藥物治療無效或對藥物產生不耐受的病人對於電氣痙攣治療能有快 速及有效的反應。個案報告：我們要報告一位37 歲男性病人患有強迫疾患及重鬱疾患，在 使用單次電氣痙攣治療後，即出現耶魯布朗強迫症評估量表明顯的改變。結論：電氣痙攣治 療合併藥物治療，可以急速改善一位重鬱症共病強迫症病人的強迫症狀，但這樣的療效仍需 要更多臨床案例，來確認我們所經驗的成功案例。

Background: Electroconvulsive therapy (ECT) is usually the last-line treatment in clinical psychiatric practice. But, many patients with major psychiatric disorders who are severely ill, resisting, or intolerant to medication, may respond more reliably and quickly to ECT. Case Report: We report a 37-year-old male patient with obsessive compulsive disorder (OCD) and major depressive disorder (MDD). He had a dramatic change of Yale Brown Obsessive-compulsive Scale score after his receiving a single treatment with ECT. Conclusion: ECT with medication showed dramatic therapeutic effect in obsessive-compulsive symptoms in a patient with MDD and OCD. But clinical observations with a larger number of cases are needed to verify our successful experience with ECT in this patient.
