The purpose of this case study is to describe nursing experience with a patient who had accepted long-term hemodialysis as an end-stage- renal disease patient. The author applied the Gordon functional assessment tool to help identify the patient ’s problems. The problems identified were excess fluid volume between hemodialysis sessions, a deficiency in knowledge of the disease and a disturbed sleep pattern. The nursing interventions were patient education, listening to the patient’s complaints and empowering the patient with self-care skills in order to assist the patient to adapt the disease process. The results of the intervention were that the patient was able to improve their fluid control ability between hemodialysis sessions, obtain an improved sleep pattern and began to experience fewer complications. |
