Wound Care--The Setup of Integrated Management Model

為改善慢性傷口病人的醫療照護，秉持全人的照護理念，成立整合性傷 口照護團隊，藉由傷口照護一整合管理模式的建蓋過程，完成了傷口照護一 整合管理模式的四大構面：標準化、稽核、軟硬體設備與效益，採取以實證 為基礎的醫療照護措施，讓病人的傷口狀況獲得改善外，更重要的是增進其 整體健康與提升生活品質。

To improve the care quality of patients with chronic wounds, the integrated wound care team was established through the process of setup the wound care-integrated management model based on the philosophy of holistic care. The wound care-integrated management model is comprised of four dimensions standard audit facility and effect. Evidence-based medical and nursing care are applied to the patients with chronic wounds. Not only the improvement of wounds, patients whole health and quality of life are also promoted.
